Instructions

I am giving you the opportunity to re-take an Exam.

  • Answer all questions and follow all directions. If approved, you will have one more attempt on the quiz/exam
  • This form needs to be completed BEFORE Tuesday, February 14th at 11:59pm if you want to re-take. No forms will be accepted after this date. 
  • You can make up the exam during lunch, before or after school before February 17th.
  • All work from the unit must be complete for approval of a retake.
  • The highest grade for a retake is a 75%!!!
